Reuters reports that McDonald's Japan has apologized to customers in the wake of another food-safety scandal.

The recent apology came after a diner found a 1 ½ -inch strip of vinyl in a Chicken McNugget over the weekend. A child was injured by a piece of plastic found in a chocolate sundae in December, and a human tooth was found in a French fry last August.

McDonald's Japan posted its first annual loss in 11 years, and has had six consecutive year of sales declines. McDonald's Corp. holds a 49.9 percent stake in the chain.
